Any false statement or omission on this application may be considered as sufficient cause for rejection of this application or for dismissal, if such false statement or omission is discovered subsequent to my employment;

I agree to completely and accurately include employment history of seven (7) years including any military service;

If I am denied employment, either wholly or partly, because of information contained in a consumer report, a disclosure will be made to me of the name and address of the consumer reporting agency making such a report;

Each Entity within Texas Health Resources will perform the pre-employment investigation to determine my suitability for employment and I authorize each Entity in Texas Health Resources to have access to any records concerning my education or employment background and any such previous employers to release information regarding my employment to the Entity in Texas Health Resources;

If I am extended an offer of employment, I will have to pass a physical examination as a condition of such employment;

If I am employed by an Entity within Texas Health Resources, such employment will be "at will" and the Entity may terminate my employment at any time;

In the event of my separation from any employment within Texas Health Resources, information concerning my employment history may be furnished to any other employer with whom I seek employment and I release each Entity within Texas Health Resources, its affiliates, successors, officers, employees and agents from all claims and liability for furnishing such information in good faith.
